Lobotomized patient Tommy Pistol is getting a checkup from nurse Lily LaBeau. Suddenly we’re sent into a place dark inside Tommy’s head, and there’s nurse Lily, bound to the ceiling of a dark abandoned ballroom, one leg bent at the knee and her arms bound behind her back, leaving her pussy exposed for mental patient Tommy. He gropes and grabs while he kneads and slaps her tits and stomach. He stuffs his stiff cock inside Lily’s pussy and fucks her hard from behind, fish-hooking her mouth for leverage. He pauses only to taste Lily’s wet pussy as she reassures him everything will be ok, but he freaks out and leaves her hanging. Back in the exam room nurse Lily listens to Tommy’s heart, only to have him sink into another fantasy. This time Lily is bound and bent over with her arms behind her back and Tommy fucks the space between her ankles. Tommy reminds her that he’s in charge and rubs his hard cock all over her face before returning to fucking her mouth. Lily pleads with him, and he screams into her pussy. He pulls her hair as Tommy rides Lily’s fuck-hole before fading back to the exam room again where Nurse Lily is asking Tommy a few questions. He slips into another fantasy where Lily is bound on her back to a wooden crate with her wrists tied to each ankle, leaving her pussy wide open for Tommy to eat. He adds the Hitachi then swaps his tongue for his cock and he fucks her hard while he grinds the vibrator into her clit and coaxes out a few orgasms. Now satisfied and really wants him Tommy fucks her pussy hard and deep until he cums all over her stomach and pussy.
